How to register

The doctors welcome new patients who live within our practice area

Follow this link to view a map of the practice boundary.

Please collect a new patient registration pack from reception (or download the documents below) and complete and return the forms as soon as possible so that you are registered before you need to book an appointment with us. If you need an urgent appointment at the time of registration please speak to the receptionist.

We ask all patients to provide proof of identity and address. Please see below the items you can present for us to verify your identity.

The practice registration pack includes a confidential questionnaire which you will be asked to complete, and if you are on repeat medication you will be invited to attend an introductory appointment with one of the doctors.

ID for all new patients registering

The registering patient should always be present to verify they are the person in the photo ID (over 18 year olds if registering with parents or 16 years old if registering without a parent)

ID from list 3 must be dated be dated within the last 3 months (if applicable).

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
